    Thank you for your interest in MD Diet & Nutrition! Our New Patient Program at our Salt Lake City location begins with a one-time payment of $175.00. You can make your payment directly at mddietclinic.com

    What’s Included in Your First Month

    Once your payment is received, a representative from MD Diet will contact you by phone or email to schedule your initial appointment with both our Nutritionist and Nurse Practitioner.

    This first appointment lasts approximately 1.5 to 2 hours and is available on Mondays, Wednesdays, and Fridays.

    Your $175 fee covers:

    • First appointment with our Nurse Practitioner
    • Nutritional one-on-one personalized
    • Four weekly Vitamin B complex injections
    • Bioanalysis and weigh-in
    • Initial blood draw

    You'll also receive weekly coaching and support to keep you motivated and on track toward your weight loss goals.

    Medication Injection Options

    MD Diet offers medically supervised injections, including:

    • Semaglutide
    • Tirzepatide

    Our Nurse Practitioner will evaluate the benefits and risks of any prescribed medications to ensure the best plan for your individual needs. Please note: Medications are not guaranteed and will only be prescribed if deemed appropriate by the medical provider.

    Cancellation Policy

    Please notify us at least 48 hours in advance if you need to cancel or reschedule your appointment.

    Cancellations with less than 48 hours' notice will incur a $40.00 fee.

No Show Policy for MD Diet Weight Loss & Nutrition

We’re excited that you’ve made the decision to get healthy & feel better by making an appointment with MD Diet Weight Loss & Nutrition’s SLC Location. We’re committed to your progress and have set aside enough time to provide you with the highest quality care. Should you need to cancel or reschedule an appointment please contact our office as soon as possible, and no later than 24 hours prior to your scheduled appointment. This gives us time to schedule other patients who may be waiting for an appointment.

Appointment Cancellation/No Show Policy for SLC Location:

Any established patient who fails to show or cancels/reschedules an appointment at the SLC location without at least 24 hours notice will be considered a No Show. The first time this happens a warning will be given. On the second or any subsequent No Show, the patient will be charged a $40.00 fee which must be paid at the time of the patient’s next office visit. As a courtesy, we make reminder calls, emails and texts for appointments. If you do not receive a reminder call or message, the above Policy will remain in effect.
