How to enter: All new patients and former patients who restart weight loss services at MD Diet Weight Loss & Nutrition’s Salt Lake City locations will be automatically entered into the sweepstakes when they call, email or schedule in person an appointment with MD Diet. Phone call, email or in person scheduling must occur after 10:00 AM MST on 7/25/18 and before 8/8/18 at 11:59 PM MST to be eligible for entry. No purchase necessary to enter. Non-MD Diet patients may also request to enter the contest by mailing a stamped envelope with a letter containing their name, email, phone number and a statement declaring that they’d like to enter the contest to 3655 State St, Salt Lake City, UT 84115. This entry must be received no later than 11:59PM MST on 8/8/18.

- Only 1 entry per person.
- Sweepstakes starts at 10:00 AM MST on 7/25/18 and ends on 8/8/18 at 11:59 PM MST.
- Only 1 winner will be selected and only 1 prize will be given away. The prize is a voucher for a 1 Night stay in a One-Bedroom Chateau Villa at the Zermatt Resort in Midway, Utah, plus 1 dinner at Z’s Steak and Chop Haus (Alcohol not included). Approximate Retail Value = $379.
- Number of eligible entries received determine the odds of winning.
- 1 winner will be selected by a random name generator on or before 8/14/18. Winner will be notified via phone or email and will have 2 calendar days to claim their prize.
- If prize is forfeited or left unclaimed a new winner will be chosen by using a random name generator.
- Winner will be announced via social media.
- Winner is responisible for paying all applicable state and federal taxes related to the prize.
